At Ziolkowski Dental we cannot allow insurance companies to tell us what quality of care we provide you.  Our office will not base your quality of care and treatment plan on dental insurance.  Instead we strive to provide the best treatment for your specific needs.  For those patients with insurance, we will always be happy to complete and submit all forms pertaining to your claim.  However, insurance companies, plans and benefits vary greatly, and not all charges may be covered or completely paid for by insurance.

          Payment is expected on the day of service, unless other arrangements have been made with Anne or Maria.  Payment can be made by cash, check, Mastercard, Visa, or CareCredit (an independent financing company).
